Physicians have probably helped patients end their own lives for centuries. Doctors who are speaking out now say the secrecy that was good practice in another era may not be so suitable for a public that is now much better informed about health care. Today's vast array of advanced medical therapies cure some patients, but they fail to work in others, leaving them to languish and suffer.

Some doctors, when they have nothing more to offer, have come to believe that they ought to help patients who wish to die. There is nothing like euthanasia to provoke angry debates among physicians, who from their profession's earliest days have been trained to heal, not to kill.

Because the account was anonymous and several medical facts were peculiar, some doctors suspected it was a hoax. Last year, however, Dr.
