She had a wide smile and flawless skin; she could have walked off the cover of Seventeen magazine. She never thought of herself as beautiful. Her older sister, Olivia McCay, said Katie was fearless and funny, but put herself under enormous pressure.

She studied for hours, all the time. While Katie was in high school, the family made two major moves. Her sophomore year they moved from Lakeland, Florida, where she grew up, to Owensboro, Kentucky. Katie fell in love with a classmate. They started talking about marriage. Katie was already contending with chronic gastrointestinal troubles and surgery.

Katie felt betrayed. When she confronted him, her family said, he broke up with her. She furiously texted and paced. Robert called their mother. Robert recalls kicking in the door and finding his little sister covered in blood, her face just gone. Five minutes later, a clinic expert says, the impulse to pull the trigger would have vanished. This is how Katie arrived more than five weeks later at the Cleveland Clinic, which was founded in by four doctors, three of whom had served together during World War I and had come home inspired by the military model of teamwork among specialists. Brian Gastman, the first clinic doctor to see Katie, wondered if she would make it.

Gastman organized a team of 15 specialists to address all her issues, from endocrinology to psychiatry.

He claims to have an attention deficit disorder personality. He specializes in head, neck, skin, and high-risk soft-tissue cancers. He removes tumors and does follow-up reconstructions.
